The temple kitchen cooks every day whether or not anyone contributes. This is a contribution to that kitchen, in your name, on a date you choose — or on the day the kitchen most needs one. You get the temple's own donation receipt in your name, and a note stating the date and the approximate number of meals your contribution covered, as the kitchen reported it. Where the trust maintains a seva list for the day, your name goes on it.

The 80G receipt and the meal count both come from the temple trust, not from us. Where a trust does not issue one, we say so on the temple before you pay rather than after.
The money goes to the temple's kitchen, not to us — you get their receipt, in your name, with the date on it. We tell you the meal count the kitchen reports, and we do not round it up.

Your full name as it should appear on the receipt. Your PAN, only if you want the 80G receipt, and only then. A preferred date, or "whenever the kitchen needs it most". An address and email for the receipt. Optionally a gotra and nakshatra where the temple reads names at the offering.
Served on the chosen date. Receipt emailed within 5 working days of the temple issuing it. The meal count follows the kitchen's own reporting, which is usually weekly.

That depends on the kitchen and the day, and we do not guess. The kitchen reports a count, usually weekly, and we pass on their number without rounding it up.
Where the temple trust issues one, yes, and you will need to give a PAN. Where a trust does not issue 80G, we say so on the temple before you pay.
The contribution goes to the temple's kitchen. The receipt you get is theirs, in your name, with the date on it.
